In one study, a variant of the ketogenic diet was applied to children with autism [51, Class III]. This diet was a modified John Radcliffe diet, which substitutes medium-chain triglycerides for some fat, but it was administered for only 4 of every 6 weeks during this 6-month trial (ie, cycles of 4 weeks “on diet” and 2 weeks “off diet” were used for the duration of the study). This group studied children on Crete, an island with a relatively isolated population and a significant number of autistic children. Behavior was rated on the standardized Childhood Autism Rating Scale (CARS) by a blinded child psychiatrist. Of the 18 children who completed the study, 2 demonstrated significant improvement (ie, CARS score reduced by > 12 points), 8 had moderate improvement (CARS score reduced by 8–12 points), and 8 showed minor improvement (CARS score reduced by 2–8 points). Children with lower starting CARS scores (less severe autism) appeared to respond better than those more severely affected. These findings should be interpreted with caution for a number of reasons. Given the geographic isolation of Crete, there may have been a strong genetic contribution to autism in this population. Methodologically, the CARS score was not designed as a longitudinal test, making its meaning in this study unclear. Additionally, intermittent administration of the ketogenic diet has not been examined in other disorders, making it difficult to compare this intervention with other studies of the ketogenic diet. Finally, any structured intervention may be associated with improved performance in patients with autism. Further study with appropriate controls (structured diet plans, vitamin administration) is needed to confirm these findings.
What is ketosis? The most important thing you’ll need to track is your carb and fiber intake. You’re trying to eat less than 20-50 grams of net carbs each day, and making your diet 70% fat. Not all calories are created equal when it comes to healthy eating and weight loss, so they’re not a focus on Keto.

Also extremely low energy nutritional approaches are used, and this energy restriction can lead to loss of lean body mass and also can impair performance as seen with dehydration (14). The practice of rapid weight loss (RWL) in weight-dependent sports is not to be underestimated. A recent review by Franchini and coworkers (10) showed that rapid weight loss has a high prevalence in combat sport practitioners such as wrestlers (60%–90%) and in judo (circa 90%). Brito et al. (3) reported a similar percentage in judo athletes (62.8%), jujitsu (56.8%), karate (70.8%), and taekwondo (63.3%). Also in weightlifting, RWL appears to be common even though no precise data are available. There are several generally harmful effects on performance resulting from such a very restricted energy intake (14) as well as harmful effects from specific methods such as dehydration using sauna or diuretics (3). Apart from increasing risks of long-term health problems, there also is an increased risk of weight gain and obesity in middle age (31); furthermore, rapid weight loss can reduce muscle performance and increase muscle damage. Dehydration can impair aerobic performance with a reduced plasma and blood volume (6) even if the actual amount of fluid loss that is necessary to have some significant effects on endurance performance is not so clear. Severe energy restriction often means a reduction of protein intake (as well as other essential nutrients) that, considering the higher needs of athletes, could induce loss of skeletal muscle mass and, in consequence, impairment of strength/power/endurance performance.

The ketogenic diet results in adaptive changes to brain energy metabolism that increase the energy reserves; ketone bodies are a more efficient fuel than glucose, and the number of mitochondria is increased. This may help the neurons to remain stable in the face of increased energy demand during a seizure, and may confer a neuroprotective effect.

The keto diet has been shown to help people lose weight in the short term; however, the long-term benefits of the diet aren't as clear, according to the Mayo Clinic. The diet is named for ketosis, which is the condition the body goes into when following the plan. In ketosis, the body uses ketone bodies, or water-soluble molecules produced by the liver and the breakdown of fatty tissue for cellular energy as opposed to sugars from ingested carbohydrates. And in some people, this results in weight-loss.
