The ketogenic diet is a high fat, moderate protein, very low carbohydrate diet that promotes rapid weight loss. It consists of about 75-80% of calories from fat, 10-15% from protein, and 5-10% from carbs. This approach to weight loss has come in and out of favor over the years in line with ever-changing beliefs about the link between dietary fat intake, weight gain, and heart disease. We are now in a period where nutrition experts and policy makers are moving away from recommendations to lower fat intake and instead are advising individuals to reduce sugar consumption as a means to lose weight and promote optimal health. This is great news! I find this a fascinating area of nutrition science and recently published a book on The Ketogenic Diet as a guide to following the diet and to discuss the role of fat and carbohydrates in weight loss.

Hot TV After a period of several days, fasting or at least a drastically reduced dietary carbohydrate content (to <20 g d-1 or 5% of daily energy intake) while maintaining usual energy intake through macronutrient redistribution, glucose reserves become depleted and no longer sufficient for either normal fat oxidation (via oxaloacetate in the Krebs cycle) or to supply energy to the brain and central nervous system (CNS).

The ketogenic diet is indicated as an adjunctive (additional) treatment in children and young people with drug-resistant epilepsy. It is approved by national clinical guidelines in Scotland, England and Wales and reimbursed by nearly all US insurance companies. Children with a focal lesion (a single point of brain abnormality causing the epilepsy) who would make suitable candidates for surgery are more likely to become seizure-free with surgery than with the ketogenic diet. About a third of epilepsy centres that offer the ketogenic diet also offer a dietary therapy to adults. Some clinicians consider the two less restrictive dietary variants—the low glycaemic index treatment and the modified Atkins diet—to be more appropriate for adolescents and adults. A liquid form of the ketogenic diet is particularly easy to prepare for, and well tolerated by, infants on formula and children who are tube-fed.

Unlike severe energy restriction, KD provides adequate amounts of energy and protein to athletes (25), avoiding protein deficiency but, at the same time, the KD, by inducing a “fasting-like” state, leads to alterations in metabolic pathways and processes such as autophagy and stress resistance. KD “mimics” energy restriction effects on 5′adenosine monophosphate–activated protein kinase (AMPK), sirtuin-1 (SIRT-1), and peroxisome proliferator–activated receptor gamma coactivator 1-alpha (PGC1α), which become activated by phosphorylation (16). In this state, PGC1α moves to the nucleus and acts as a transcription factor, increasing the expression of genes that code for proteins involved in fatty acid transport, fat oxidation, and oxidative phosphorylation. The activation by phosphorylation of PGC1α may occur in several ways involving AMPK, calcium calmodulin–dependent protein kinase, and p38 mitogen-activated protein kinase pathways. SIRT-1–mediated deacetylation also can cause activation. AMPK1 can work in two ways, either by activating PGC1α by phosphorylation or else directly by promoting the expression of enzymes involved in skeletal muscle oxidative effects and metabolism. This is supported by observations that, in obese subjects, the skeletal muscle is less oxidative and has, during fasting, lower AMPK activation (9). At the same time, AMPK activation also inhibits mammalian target of rapamycin (mTOR ) signaling, which is an important factor involved in regulating muscle mass (32).

Class I histone deacetylases (HDACs) can influence gene expression and transcription, which causes problems in the body. One ketone body, d-β-hydroxybutyrate (βOHB), can inhibit the HDAC [R]. βOHB changes histone acetylation and gene expression, which reduces HDAC activity. This promotes stress resistance in the kidney. βOHB protects the brain and enhances the resistance of nerve cells to oxidative damage [R].
